Medication error is the most common and consistent type of error occurring in hospitals. This article attempts to explore the ethical issues relating to the nursing management of medication errors in clinical areas in Macau, China. A qualitative approach was adopted. Seven registered nurses who were involved in medication errors were recruited for in-depth interviews. The interviews were transcribed and analyzed using content analysis. Regarding the management of patients, the nurses acknowledged the mistakes but did not disclose the incidents to (...) patients and relatives. Concerning management of the nurses involved by senior staff, most participants experienced fairness, comfort and understanding during the process of reporting and investigation. The ethical issues relating to the incidents were discussed, particularly in the Chinese context. There is a need for further study relating to the disclosure of medication incidents to patients and some suggestions were made. (shrink)

The solution of the outer multiplicity problem in the tensor product of U(3) irreducible representations (irreps) developed by Biedenharn et al.(1–7) and realized through the well-known Draayer-Akiyama (DA) computer code(8) is extended to the quantum algebra Uq(3). An analytic formula for special stretched Uq(3) Wigner coefficients, $$\left\langle {(\lambda _1 \mu _1 ) H_1 , (\lambda _2 \mu _2 ) \varepsilon _2 \Lambda _2 m_2 \left| { (\lambda _3 \mu _3 ) H_3 } \right.} \right\rangle _{\max }^q $$ is derived using (...) a projection operator method.(9–10) In this expression Hi denotes the highest weight vector of the (λiμi) irrep; the subscript “max” means coefficients corresponding to a unit tensor operator with a maximal characteristic null space; and q is the usual quantum label so the standard U(3) Wigner coefficient, which is required in the DA code, can be obtained in the q→1 limit of the theory. To illustrate the theory, some Uq(3) Wigner coefficients for the tensor product (22)×(22) are calculated. The Einstein-de Broglie soliton concept is applied to simulate stationary states of an electron in a hydrogen atom. According to this concept, the electron is described by the localized regular solutions to some nonlinear equations. It is shown that the electron-solilon center travels along some stationary orbit around the Coulomb center. The electromagnetic radiation is absent as the Poynting vector has non-wave asymptote O(r −3)after averaging over angles.

The simplest nonlinear spinor field equation admitting regular stationary solutions is considered. Following a causal interpretation of quantum mechanics, given by de Broglie in his double solution theory, these regular solutions must be regarded as describing the internal particle structure. Using this spinor field model, an attempt is made to give a statistical description of one-particle experiments by means of a Gibbsian assemblage method. It is shown that in the limiting case of pointlike nonrelativistic particles this method is equivalent to (...) the quantum mechanical one, that is, the probability amplitude can be introduced satisfying all postulates of quantum mechanics. Previous studies suggested that facial attractiveness perception can be increased with facial skin homogeneity improving; and human’s facial change detection increases along with facial skin homogeneity increases. However, it’s unknown whether a face can be perceived prettier than it did before while still being considered as physically the same. It is possible that these two kinds of cognitive-aesthetic processing may have separate mathematical functions in psychophysical studies. In other words, human’s facial attractiveness differentiation may be more sensitive than facial change (...) detection. In this current study, we explored the above questions. Using three types of psychophysical techniques to manipulate facial skin homogeneity, we measured how participants’ sensitivity to facial skin homogeneity and attractiveness change. Results showed a linear function curve for facial physical change detection and a logarithmic function curve was drawn in the forced-choice technique, which was the most sensitive one, indicating that participants can judge a face prettier than before without being aware of it has physically changed. Besides, two linear function curves were shown in the same/different technique and a rating technique. Taken together, this current study revealed that facial attractiveness can be enhanced and discriminated by improving facial skin homogeneity, without being realized by people with conscious awareness that the face has been changed. Recently discovered transcription‐independent features of p53 involve the choice of DNA damage repair pathway after PARylation, and p53's complex formation with phosphoinositide lipids, PI(4,5)P2. PARylation‐mediated rapid accumulation of p53 at DNA damage sites is linked to the recruitment of downstream repair factors and tumor suppression. This links p53's capability to sense damaged DNA in vitro and its relevant functions in cells. Further, PI(4,5)P2 rapidly accumulates at damage sites like p53 and complexes with p53, while it is required for ATR recruitment. (...) These findings help explain how p53 and PI(4,5)P2 maintain genome stability by directing DNA repair pathway choice. Additionally, there is a strong correlation between p53 sequence homology, genome mutation rates as well as lifespans across various mammalian species. Further investigation is required to better understand the connections between genome stability, tumor suppression, longevity and the transcriptional‐independent function of p53. (shrink)

BackgroundThe Public Health Service Increased Risk designation identified organ donors at increased risk of transmitting hepatitis B, hepatitis C, and human immunodeficiency virus. Despite clear data demonstrating a low absolute risk of disease transmission from these donors, patients are hesitant to consent to receiving organs from these donors. We hypothesize that patients who consent to receiving offers from these donors have decreased time to transplant and decreased waitlist mortality.MethodsWe performed a single-center retrospective review of all-comers waitlisted for liver transplant from (...) 2013 to 2019. The three competing risk events (transplant, death, and removal from transplant list) were analyzed. 1603 patients were included, of which 1244 (77.6%) consented to offers from increased risk donors.ResultsCompared to those who did not consent, those who did had 2.3 times the rate of transplant (SHR 2.29, 95% CI 1.88–2.79, p < 0.0001), with a median time to transplant of 11 months versus 14 months (p < 0.0001), as well as a 44% decrease in the rate of death on the waitlist (SHR 0.56, 95% CI 0.42–0.74, p < 0.0001). All findings remained significant after controlling for the recipient age, race, gender, blood type, and MELD. Of those who did not consent, 63/359 (17.5%) received a transplant, all of which were from standard criteria donors, and of those who did consent, 615/1244 (49.4%) received a transplant, of which 183/615 (29.8%) were from increased risk donors.ConclusionsThe findings of decreased rates of transplantation and increased risk of death on the waiting list by patients who were unwilling to accept risks of viral transmission of 1/300–1/1000 in the worst case scenarios suggests that this consent process may be harmful especially when involving “trigger” words such as HIV. The rigor of the consent process for the use of these organs was recently changed but a broader discussion about informed consent in similar situations is important. (shrink)

Inspired by Hintikka’s ideas on constructivism, we are going to ‘effectivize’ the game-theoretic semantics for independence-friendly first-order logic, but in a somewhat different way than he did in the monograph ‘The Principles of Mathematics Revisited’. First we show that Nelson’s realizability interpretation—which extends the famous Kleene’s realizability interpretation by adding ‘strong negation’—restricted to the implication-free first-order formulas can be viewed as an effective version of GTS for FOL. Then we propose a realizability interpretation for IF-FOL, inspired by the so-called ‘trump (...) semantics’ which was discovered by Hodges, and show that this trump realizability interpretation can be viewed as an effective version of GTS for IF-FOL. Finally we prove that the trump realizability interpretation for IF-FOL appropriately generalises Nelson’s restricted realizability interpretation for the implication-free first-order formulas. Cyclin-dependent kinase 5 has been implicated in Alzheimer's disease pathogenesis. Here, we demonstrate that overexpression of p25, an activator of cdk5, led to increased levels of BACE1 mRNA and protein in vitro and in vivo. A p25/cdk5 responsive region containing multiple sites for signal transducer and activator of transcription was identified in the BACE1 promoter. STAT3 interacts with the BACE1 promoter, and p25-overexpressing mice had elevated levels of pSTAT3 and BACE1, whereas cdk5-deficient mice had reduced levels. Furthermore, mice with a (...) targeted mutation in the STAT3 cdk5 responsive site had lower levels of BACE1. Increased BACE levels in p25 overexpressing mice correlated with enhanced amyloidogenic processing that could be reversed by a cdk5 inhibitor. These data demonstrate a pathway by which p25/cdk5 increases the amyloidogenic processing of APP through STAT3-mediated transcriptional control of BACE1 that could have implications for AD pathogenesis.
